NC DMV form MVR-614 previously referred to as the Affidavit of Proof of Residency has been revised and will now be referred to as the Affidavit of Principally Garaged Vehicle.

Please note that item number (7) requires the applicant to provide their permanent home state address. Additionally, item number (8) requires the attachment of a photocopy of the customer’s home state driver’s license or Identification card. All Photocopies must be clear and all pertinent drivers license or special identification issuance and expiration information legible.

The revised MVR-614 reflecting the revision date of July 2011 is now required (as of September 1, 2011)
